A brass plaque with embossed lettering and matt black surround within the church of St. Leonard's in Walton-le-Dale. The names of 54 men from the parish are arranged in 5 columns together with rank and regiment or service.

WALTON-LE-DALE CHURCH WAR MEMORIAL. At St. Leonard's Church, Walton-le-Dale, on Sunday afternoon, a memorial service for the men of the parish who fell in the war, will be held. The Rev. C. Egerton Chadwick M.C. M.A., chaplain at Fulwood Barracks, who was in the retreat from Mons, will give an address. He was deputy-assistant Chaplain-General to the British forces in France. At the service a bronze tablet containing the names of the 54 men of the parish who fell in action will be unveiled by Sir James de Hoghton, Bt. The tablet, which measures seven and a half feet and cost about £260, has been met by subscriptions from the parisioners. The tablet will be dedicated by the vicar, the Rev. E. J. M. Davies. Four R.F.A. trumpeters from Fulwood Barracks will sound the "Last Post" and "Reveille" after the dedication. LANCASHIRE DAILY POST, Friday 22 October, 1920
