The memorial takes the shape of a brass plaque mounted upon a wooden board. It bears the names of eight local Walton-le-Dale men who died in the Great War, as well as the names of three men who died in World War two.
The memorial was originally sited in the Walton-le-Dale Methodist Church on Church Brow in Walton-le-Dale but was moved to Trinity Church in Hoghton when Methodist churches were reorganised in this area.
